Weekly dose vs. dose per injection

TRT prescriptions are often written as a weekly total, such as 140 mg per week, but some prescribers split that into two or more smaller injections to keep blood levels steadier between shots. The calculator works per injection, so divide the weekly total by your number of injections first:

Cypionate vs. enanthate

Both are long-acting testosterone esters dosed the same way by milligram. Cypionate is the most common in the US and is typically suspended in cottonseed oil; enanthate is usually in sesame oil, which matters for anyone with a sensitivity to one of them. The calculation is identical: mL = mg ÷ mg/mL. The difference that changes your draw is the concentration, so check the label whenever you switch products or pharmacies.

How many mL is 100 mg of testosterone cypionate?

At the most common strength, 200 mg/mL, 100 mg is 0.5 mL (50 units on an insulin syringe). At 100 mg/mL it is 1 mL. Testosterone enanthate at 250 mg/mL would be 0.4 mL.

How many mL is 200 mg of testosterone?

At 200 mg/mL, 200 mg is exactly 1 mL. At 250 mg/mL it is 0.8 mL, and at 100 mg/mL it is 2 mL, which is more than an insulin syringe holds.

How do I split a weekly testosterone dose into two injections?

Divide the weekly dose by the number of injections. 140 mg a week split into two injections is 70 mg each; at 200 mg/mL that's 0.35 mL, or 35 units. Enter the per-injection dose and set injections per week to 2 in the calculator. Only change your injection frequency if your prescriber has told you to.

Can I use an insulin syringe for testosterone?

Many TRT patients inject subcutaneously with insulin syringes, which is why the calculator shows units. Intramuscular injections use a longer needle on a 1–3 mL syringe, so go by the mL figure. Which route and syringe you use is your prescriber's call.

How long will a 10 mL vial of testosterone last?

A 10 mL vial at 200 mg/mL holds 2,000 mg. At 100 mg a week that's 20 weeks; at 200 mg a week, 10 weeks. Multi-dose vials also have a discard date once opened, which your pharmacy label specifies.
