What to look for

A prescription from a licensed clinician, filled by a licensed pharmacy

Every legitimate product in this category requires a prescriber evaluation and is dispensed by a state-licensed 503A compounding pharmacy or a registered 503B outsourcing facility. 'Research peptide' websites that sell vials labelled 'not for human use' without a prescription are selling unregulated chemicals with no sterility, potency, or identity guarantees — no clinician, no pharmacy, no accountability.

503A vs. 503B sourcing

503A pharmacies compound patient-specific prescriptions and are overseen primarily by state boards of pharmacy. 503B outsourcing facilities register with the FDA, follow current good manufacturing practice (cGMP), and are inspected by the agency. Both are legal routes; 503B generally means more consistent quality control for sterile injectables. Providers that name their pharmacy and its type are easier to hold to account.

Which peptides are actually compoundable

In 2023 the FDA placed a number of popular peptides — including BPC-157, thymosin beta-4 fragments (TB-500), and several growth-hormone secretagogues — in Category 2 of its 503A bulk-substance list, meaning the agency identified significant safety risks and pharmacies should not compound them. Several nominations were later withdrawn and those substances removed from Category 2, which did not make them compoundable: a substance on no list is generally ineligible for 503A compounding unless it is a component of an approved drug or has a USP monograph. A provider openly prescribing BPC-157 injections is operating outside that framework.

Baseline labs and a defined goal

Growth-hormone secretagogues (sermorelin, tesamorelin) should be preceded by IGF-1 and glucose testing and followed up with repeat labs. Rapamycin and metformin warrant lipid, glucose, kidney, and blood-count monitoring. A program that ships injections with no labs and no stated clinical goal is selling a product, not providing care.

Honest evidence framing

Look for providers that distinguish FDA-approved uses from off-label and wellness uses, and that describe expected effects in measured terms. Claims of 'reversing aging', 'cellular repair', or guaranteed energy gains are marketing language that no trial in this category supports.

All-in monthly cost

Pricing commonly splits into a membership or consult fee, the medication, supplies (syringes, bacteriostatic water), and labs. Injectables in this category typically run from roughly $100 to $400 per month each, and stacked protocols multiply quickly. Price the full year, including labs, before committing.

Frequently asked questions

What is the best longevity and peptide clinic?

▾

Measured ranks #1 on this list with a score of 7.9/10 and plans from $79/mo. Scores weigh trust and safety, value, patient experience, product range and reputation.

What is the cheapest longevity and peptide clinic?

▾

Of the providers on this list, AgelessRx has the lowest starting price at $25/mo (Metformin). Starting prices often rise with dose or after an intro period, so compare the ongoing price too.

How much does a longevity and peptide clinic cost?

▾

Across the 64 providers we compared, starting prices range from $25 to $325 per month, with a median of $119. 25 of them run an intro offer or first-month discount.

How are these providers ranked?

▾

By our category score: trust and safety (30%), value (25%), patient experience (20%), product range in this category (15%) and reputation (10%), with a small bonus for specialists. The full methodology is published on our methodology page.

Is there good evidence that NAD+ injections slow aging?

▾

Not in humans yet. NAD+ levels decline with age, and raising them extends healthspan in some animal models. Human studies — mostly of oral precursors such as NR and NMN — show that blood NAD+ levels rise, but effects on clinical outcomes have been small or inconsistent. Injectable and IV NAD+ have very little controlled trial data. It is reasonable to treat NAD+ as an experimental wellness therapy rather than an established treatment.

What is the difference between sermorelin, tesamorelin, and growth hormone?

▾

All three act on the growth hormone axis. Recombinant human growth hormone (somatropin) replaces GH directly and is tightly regulated — prescribing it for anti-aging is not a permitted use under federal law. Sermorelin and tesamorelin are GHRH analogues that stimulate the pituitary to release its own GH. Tesamorelin (Egrifta) is FDA-approved to reduce abdominal fat in HIV-associated lipodystrophy; sermorelin's branded product was discontinued in 2008 and it is now available only compounded. Neither is approved for anti-aging.

Why can't I get BPC-157 from a telehealth provider?

▾

Because the FDA has told compounding pharmacies not to make it. BPC-157 has no approved drug, no USP monograph, and minimal human safety data, and the agency listed it among bulk substances raising significant safety concerns in 2023. Sites selling it as a 'research chemical' are not dispensing medicine, and product-testing surveys of such sites have repeatedly found mislabelled potency and contamination.

Is rapamycin for longevity legitimate?

▾

Rapamycin (sirolimus) is an FDA-approved transplant drug. It is the most consistently effective lifespan-extending compound in mouse studies, which is why longevity clinicians prescribe it off-label in low, intermittent doses. Human evidence is early: small trials suggest intermittent low doses are reasonably tolerated, but no study has shown it extends human lifespan or healthspan. It is a legal off-label prescription, and it is also an immunosuppressant that requires monitoring.
