Wegovy After 1 Month: What to Realistically Expect

The first month on Wegovy is mostly about getting used to the medication. You start on the lowest dose (0.25 mg once a week), appetite usually starts to quiet down, and weight loss begins, but it's modest. The bigger changes come in months two through six as the dose climbs.

The first month, week by week

Wegovy's label sets the starting schedule:

If a step isn't tolerated, the label allows delaying the next increase by 4 weeks. The newest label also permits a higher 7.2 mg dose (Wegovy HD) after at least 4 weeks at 2.4 mg.

How much weight people lose early on

  • Month one: In the STEP 1 trial, weight loss was already visible at the first check at week 4, but this is the 0.25 mg phase, and changes are small compared with later months. Expect your first month to be about noticing less hunger and smaller portions more than the scale.
  • Month three: In a Mayo Clinic study of 175 people using semaglutide in routine care, average loss was 5.9% of body weight at 3 months.
  • Month six: The same study found 10.9% at 6 months.
  • Long term: In STEP 1, average loss was 14.9% at 68 weeks, versus 2.4% on placebo.

Side effects to expect

Stomach side effects are common and cluster around dose increases. Across the adult trials on the label:

Severe GI reactions were uncommon (4.1% vs 0.9% on placebo). Eating smaller, lower-fat meals, stopping when you feel full, and staying hydrated help most people. Persistent vomiting or severe abdominal pain needs a call to your prescriber.

The Wegovy pill's first month

The Wegovy pill (oral semaglutide 25 mg), approved in December 2025, has its own schedule: 1.5 mg daily for the first 30 days, then 4 mg, 9 mg and 25 mg. In the OASIS 4 trial, average loss was 13.6% at 64 weeks, and side effects were similar to the injection. Because it starts at a different dose, its first month isn't directly comparable with the injection's.

Why is the first Wegovy dose so low?

What are the most common side effects in the first month?

Nausea is the most common side effect overall (44% vs 16% on placebo across trials), followed by diarrhea, vomiting and constipation. They're most frequent during dose increases and often ease as your body adjusts. Eating smaller meals and stopping when full helps.

Is the first month different on the Wegovy pill?

Should I be worried if I don't lose much weight in month one?

Not usually. The starting dose is designed for tolerability, and response builds as the dose rises. Talk to your prescriber if you have little appetite change after reaching the higher doses, or if side effects are making it hard to eat or drink.
