What to look for

What pt-141 (bremelanotide) is

PT-141 is the research name for bremelanotide, a peptide that acts on the brain to increase sexual desire. Brand-name Vyleesi, an injection used as needed, was FDA-approved in 2019 for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD). Telehealth providers also prescribe compounded PT-141 injections or nasal sprays, including for men, which is off-label.

How it works

Unlike ED medications such as sildenafil, which increase blood flow, bremelanotide works in the brain. It activates melanocortin receptors (mainly MC4R) in areas involved in sexual desire and arousal. In Vyleesi's phase 3 trials in premenopausal women, it produced small but statistically significant increases in desire and reductions in distress compared with placebo; it did not significantly increase the number of satisfying sexual events. Studies in men, mostly earlier and smaller, suggested it can help erections, including in some men who don't respond to sildenafil, but it is not approved for men.

What to expect

Taken as needed, not daily. Vyleesi is injected under the skin at least 45 minutes before sexual activity, and the label limits use to one dose per 24 hours and eight doses per month. Effects can last several hours. Women who see no improvement after 8 weeks of trying it should stop, according to the label.

Who it's not for

People with uncontrolled high blood pressure or known cardiovascular disease; Anyone who is pregnant; People taking naltrexone by mouth, since bremelanotide can lower its levels; Men with ED who haven't tried FDA-approved options first (off-label use).

Is PT-141 the same as Vyleesi?

▾

Same molecule, different products. Vyleesi is the FDA-approved, brand-name bremelanotide autoinjector for premenopausal women with HSDD. 'PT-141' usually refers to compounded bremelanotide from telehealth and peptide clinics, whose strength and quality depend on the compounding pharmacy.

Does PT-141 work for men?

▾

Possibly, but it is off-label. Earlier studies in men showed improved erections, including some who didn't respond to sildenafil, but larger trials and FDA approval focused on women. Because it works on desire rather than blood flow, some men use it alongside an ED medication; ask your prescriber before combining them.

How is PT-141 different from Viagra or Cialis?

▾

Sildenafil and tadalafil increase blood flow to the penis and need sexual stimulation to work. Bremelanotide acts in the brain to increase desire and arousal. They treat different problems, and the ED medications have a much larger evidence base.

Why does PT-141 cause nausea?

▾

Nausea is the most common side effect, reported by about 40% of women in Vyleesi's trials, mostly with the first dose. It usually eases with later doses. Some prescribers start with a lower dose or suggest an anti-nausea medication.
