Enclomiphene is the trans-isomer of clomiphene, a selective estrogen receptor modulator (SERM). How it works

Enclomiphene blocks estrogen receptors in the hypothalamus and pituitary, so the brain perceives lower estrogen and increases output of LH and FSH. Higher LH signals the testes to produce more testosterone; higher FSH supports sperm production. Unlike TRT, which suppresses the body's own production, enclomiphene works through it — which means it only helps men whose testes can still respond (secondary hypogonadism). Phase 3 trials showed it raised testosterone into the normal range while maintaining sperm counts, but the FDA found the trials did not demonstrate clinical symptom benefit.
